This form should be used to submit proof of hours volunteered at a CDCT function. Individuals are required to volunteer for three hours in order to be eligible for any CDCT Award.
You may volunteer at any CDCT shows or clinics, attend board meetings, serve on the board of directors or any of its committees, help with publicity, or help organize clinics or demonstrations.
Print and copy this form as needed. Return completed form to the membership secretary by December 31 of the competition year.
